A personal code of conduct is a set of explicit commitments about how you will behave, especially under pressure. It is not aspirational. It is declarative. Not 'I want to be honest' but 'I tell the truth even when it costs me.'

Start by identifying your most common integrity failures. Write them as behavioral standards, not qualities. Keep it short — five to ten commitments you will actually live by.
